Booth Application

Shelton Day is October 7, 2018

Time: 11:00 AM – 5:00 PM (booths are on Howe Ave. which is closed for the day.)
Booth Size: 12 feet deep by 15 feet wide
Set-up time: No Later than 10:00 am
Breakdown time: 5 pm Depart by 6 pm

After submitting the on-line application below, send your check made payable to Derby-Shelton Rotary to  Shelton Day, c/o Al Patuzzi 2 Astor Drive Shelton, CT 06484. Booth locations will be assigned on a first-come, first served basis. Vendors from last year will have the right to their location from last year, as long as their application and check are received by June 1. Once Booths are assigned, there will be no changing of location.

All food vendors must have a valid Health Department License. Contact the Naugatuck Valley Health Department for information at: 203-881-3255.

Any vendor exceeding their allotted space will be asked to leave without a refund. No electricity, tables or chairs will be supplied. If required, you must make your own arrangements. All vendors are responsible for cleaning up their area at the end of the day. Any vendor dumping oil or other cooking materials will be reported to the police.
Vendors may not use a public-address system.

Below you will find the vendor application for 2018 which you simply fill out and submit. Once you have submitted the application, send your check made out to the Derby-Shelton Rotary Club to:  Shelton Day, c/o Al Patuzzi 2 Astor Drive Shelton, CT 06484. Your booth is not reserved until your check is received!
